The Mt. Washington Pediatric Neonatal Transitional Care Follow-up Clinic has been designed to provide a comprehensive, multidisciplinary post-discharge program for at-risk infants. The program provides ongoing clinical monitoring and services from a specialty team consisting of a Neonatologist, Nurse, Psychologist and Rehabilitative Therapist.

Patients served by this clinic include, but are not limited to, high-risk infants, former premature infants (otherwise not considered high-risk) and newborns (otherwise not considered high-risk) with diagnoses of any of the following:

  • Infants <_ 1250 grams
  • Intraventricular hemorrhage – grade III or IV
  • Periventricular leukomalacia
  • Post-hemorrhagic hydrocephalus
  • Meningitis with abnormal neurologic exam
  • Seizures, secondary to hypoxic ischemic encephalopathy
  • Prolonged or complicated respiratory failure
  • Former Extracorporeal membrane oxygenation (ECMO) dependency
  • Congenital Diaphragmatic Hernia
  • Hyperbilirubinemia with abnormal neurologic exam
  • Recurrent apnea, discharged home on an apnea monitor
  • Chronic Lung Disease/Bronchopulmonary Dysplasia
  • Necrotizing enterocolitis requiring surgical intervention

Core services include the following:

  • A comprehensive history and physical examination
  • Adjustment of chronic NICU medications
  • Monitoring of growth
  • Review of development

Additional services may include referrals for:

  • Oxygen adjustment
  • Neurodevelopmental assessments
  • Visual and auditory assessments
  • Rehabilitation evaluations and management
  • Cognitive assessments
