Weigh Smart Enrollment
There are three steps to enroll in the Weigh Smart® Program.
1. Fill out the New Patient Information Form and send to us.
Return Completed Forms
To return completed forms, you may:
1. Fax them to: 410-578-2654.
2. Email to: [email protected]. Please note email is not secure so your child's health information could be compromised unless encrypted.
3. Mail to:

Once the forms are received, our program coordinator will call you to set up your child's evaluation appointment.
2. Before your child's appointment, get the following lab work through your pediatrician's office.
- Serum lipid panel*
- Hemoglobin A1C*
- Blood glucose
- AST
- ALT
- Vitamin D
*Weigh Smart Jr. patients (ages 2-7) only need to have these two tests completed.
3. Before your child's appointment, contact your child's pediatrician to ask them to send us your child's last clinic note, labs and growth charts by fax to 410-578-2654.
